Targeted Health Promotional Programs and Assessment
Problem Statement: Adequate healthcare cannot be achieved without efficient and effective communication between patients and providers. Patients who have difficulty communicating, will continue to execute unhealthy life styles and experience poor health, pain, premature deaths, consequential medication errors, dissatisfaction and incentives to litigate caused by a host of, often preventable, sentinel events.
• Surgery: The New England Journal of Medicine released a study on re-hospitalized patients stating that approximately 25% are readmitted within 30 days. Readmission rates are directly related to a breakdown in post-discharge instruction and care.
• Primary Care: Primary care physicians provide 52% of all ambulatory care visits, 80% of patient visits for hypertension, and 69% of visits for both chronic obstructive pulmonary disease and diabetes, yet they make up only one-third of the U.S. physician workforce. Compelling evidence at the state, county, and local level confirm that primary care improves health outcomes, increases quality, and reduces health care costs. .Despite the availability of such evidence, the United States has done little to ensure that the supply of primary care physicians is sufficient to meet current and future needs.
Solutions: ClairtyVision (CV) and T1Visons (T1V), through means of proprietary software and interactive “LCD Touch Screen” monitors, has developed technical programs that deliver healthcare education, services and screening. Through a collaborative approach, we deliver prerecorded scripted media of the right topic, at the right time and the right place. Conjoining the process and with structure, we generate operational efficiencies, bring forth preventative opportunities and deliver consistent and valid information to the patient. As a result, risks are reduced through standardize health prevention collaboration and commutation between the provider and the patient.
Video clips are produced using the actual providers and include health specific topics, protocols and site specific information that will educate their patients’ about possible health risks proactively. This educational media presentation empowers the patient to ask educated questions surrounding their specific wellness areas & demographics and encourages a proactive self-care and medical maintenance initiatives.
Once produced, the video is placed into a software driven decision matrix that is hosted on a centralized server. At the work station, allied health care professionals drill down to the specific educational elements to be shared with the patient. With a touch of the screen, media is sent to the patient who is often waiting idly in exam rooms, patient beds and pre-operative bays.
Though innovative approaches in health prevention and use of technology the ClarityVision Media Package increases the efficiency and effectiveness of our limited supply of medical providers; increases the quality and consistency in what is communicated; reduces errors in medical advice often rendered by second parties; provides the ability to decrease payor cost through awareness and proactive healthcare prevention; provides the ability for providers to increase revenues through preventive exams and treatment, and most importantly can improve the health patients.
